In many cases the problem arises directly at the skin’s level: inflammation, possibly due to contact with an irritant substance, dryness of the skin (xerosis), typical of the elderly and of those suffering from atopic dermatitis. Or dermatological diseases that are associated with skin manifestations such as solar erythema, characterized by small lesions, chronic urticaria or contact dermatitis, which is also manifested by desquamation. Or the scab, caused by a mite that lives under the skin and which can give itching as a single symptom, especially at night.

When the skin is not problematic

But itching can also be born “away” from the skin, involving the whole body and persisting for a long time. It happens when the pathology that causes it is not dermatologic, such as chronic kidney disease, liver disease, diabetes, thyroid problems, some forms of cancer or neurological diseases. In some cases itching can be a mere psychological discomfort.

How to recognize an allergy

One thing is certain: itching, if it is not associated with any skin manifestation, is never an allergic nature. When it comes to skin symptoms similar to a dermatitis, the cause can be a contact allergy. It can be used for personal hygiene or creams, hair dyes or detergents. The first move, then, is to understand where the problem arises.

“To determine whether a substance can cause skin inflammation with an allergic mechanism, there is a method called patch test,” says Patrizia Bonadonna, specialist in allergology and clinical immunology at Verona’s integrated university hospital. A small amount of substance is placed in plastic or aluminum cells attached to a patch (or patch) that is applied to the back of the skin and is maintained for 48 to 72 hours, after which the doctor removes it and verifies the result.

In addition to a contact allergy, itching can also be caused by an allergy to food. In this case the symptoms are instantaneous: mouthpiece, throat and ears, hives, swelling in the face, especially around the eyes, lips, tongue and palate. In some cases the reaction may be very violent with the appearance of respiratory difficulties.

The most common allergens are eggs, milk, crustaceans, grain, peanuts, hazelnuts and hazelnuts, cherries, peaches, plums, apricots, kiwis, carrots, raw potatoes, celery and legumes.

“It is important not to confuse an allergy with an eating intolerance. A first distinction is in the appearance of the symptoms: in the first case – Bonadonna clarifies – they are immediate and can also be caused by the intake of small amounts of food, while in the second the picture is more complex and manifests in hours. In addition, in the case of intolerances, the symptoms are usually gastrointestinal (diarrhea, difficulty digesting, swelling and abdominal pain) or somewhat aspecific, such as headache. “
